Midam District Required Documents 2024-2025
The deadline for the documents is July 30, 2024. Failure to complete and return the documents will cause your association to be suspended until the are completed and return to me.
Please make sure you put your USAH Association code on each document. If you have any questions, please contact me or your State VP.
The Tier I and II Intension document is for youth travel teams only (National Bound)
